How to Determine Your Insurance Benefits for Physical Therapy

We are in network with most major Texas commercial insurance including Medicare and Medicaid. 

​

We accept most health insurance. Filing insurance claims is a courtesy that we extend to our patients, though all charges are your responsibility from the date services are rendered. Your insurance is a contract between you, your employer, and the insurance company. We are not a party to that contract. Before your visit, it is your responsibility to contact your insurance company to verify that we are participants in your particular plan and that the services you intend to receive are covered.

Co-payments, estimated co- insurances, estimated deductibles, fees, and balances are due at time of service.

If prior authorization/referral is required by your insurance carrier, it is your responsibility to obtain this from your primary care physician. Failure to do so may result in denial of your claims and you will be held responsible for all non-covered charges.

Tips to learn more about your rehabilitation insurance benefits:

​

  1. Call the toll-free customer service number on your insurance card. Select the option that will allow you to speak with a customer service provider, not an automated system.

  2. Ask the customer service provider to quote your physical therapy benefits. These are also known as rehabilitation benefits which can also include therapies such as occupational therapy, speech therapy, and even massage therapy.

  3. If your plan requires pre-authorization, please allow one week to process before your initial visit. Let our office know this information immediately and whether a form is required to fill it.
